Forms for current volunteers or new volunteers that have completed orientation
Current volunteers need to resubmit their Background Check forms May 15 - June 30 every year

Adult Volunteers:
If you are an adult age 18 years or older:
Please print and fill out the following forms and save them for upload. All information must be in black font or pen, and the format unchanged. Please date and sign the form(s).
Background Check Form (all adults) and
Media and Liability Release & Equine Waiver Form (if volunteering at the farm)
Complete the Active Volunteer Submission Form HERE and upload your completed forms therein so that we may obtain clearance for your onsite work
Minor Volunteers:
If you are under the age of 18 years old:
Please have a parent/guardian complete the following form on your behalf:
* Media and Liability Release & Equine Waiver Form (if volunteering at the farm)
Complete the Active Volunteer Submission Form HERE and upload your completed Media/Equine forms. You do not need to complete a Background Check Form.
